If you were unable to complete the Supplemental Training Request Form when initially requesting a course for any reason (ex... browser issues, timeout, form would not submit) you can easily locate the form, complete and submit by following the instructions below.


How to locate and complete the CSTI Supplemental Training Request Form:

1. Login at CSTI.ORG (Alternately
https://csti-ca.csod.com/)
2. Navigate to: Main Menu > Home Universal Profile > Transcript
3. Ensure the "Active" Tab is selected.

4. The Active Transcript area lists all upcoming training you have either requested or are registered for. Locate the training you are attempting to register for, it will have the status set to "Pending" below the course name.
5. Click the blue "View Training Details" button. ( See screen shot ).

6. Scroll part way down the page until you see the " Request Form" section. Click the small paper pencil edit icon next to the form name * Your form name may be different than the screen shot below.

7. The form will open. Fill out all fields in the form and select SUBMIT. * There are areas of the form which allow numbers only, no dashes or other spaces.


Once the form is successfully submitted, our registrars will review your request and you will be notified via email of our decision.
